Multidisciplinary Treatment Team Approach
A Multidisciplinary Treatment Team works closely with the client to achieve optimal functional gains. Clients, families, external case managers, guardians, legal representatives, school liaisons, and other stakeholders are encouraged to participate in routine treatment/educational team meetings to assure goals, objectives and strategies are targeting desired outcomes.
All parties work together to develop and implement an appropriate, client-centered educational and transitional care plan. All team members are encouraged to provide ongoing feedback to the Team so the care plan can be adapted to focus on changing needs. Support services are provided by licensed rehabilitation therapists, behavior analyst, specialists, and doctors who specialize in brain injury and disease. Training in compensatory strategies is provided to the client, their family members and significant others, as well personal care attendants.
Individual Education Planning (IEP) / Transitional Education
The transitional education program works with home school systems to implement IEP’s for children, adolescents, and young adults who require transitional education and behavior services as per Federal IDEA / FAPE regulations.
