If you are a family, case manager or payor and you are unable to travel to visit our programs, we will send you the equipment next day mail so you can have a video meeting with our treatment team members.We can also send a program representative or clinical evaluator to meet with you in your office, residence, or destination of your choice.

If you are a discharge planner in a hospital, IRF, LTACH, SNF, ALF, Behavioral Health Center, State Agency or other program, please fax your referral and/or medical records and patient demographics to 866-821-8885 . Upon receipt, we will quickly determine if the patient is a candidate for one of our programs. We will contact you immediately and send a clinical evaluator to your facility to assess the case and meet with your treatment team, the patient, and family.
